$75/month ........ one office visit or one home visit as needed

For toenail trimming and callus care, shoe evaluation, and skincare only

General Plan

$150/month ......... Concierge level care and direct care provided by Dr. Patel for injury evaluation, surgical questions, prescription refill requests, and more.

*In-network insurance can be billed for covered services in addition to membership fee

New Patient Office Visit -- $225
Dr Patel will go listen to your history, symptoms, and needs to formulate a treatment plan
(INCLUDES XRAY OR ULTRASOUND IF NEEDED)

Followup Office Visit (simple) -- $75
Simple Followups include evaluating healing from recent minor procedures, results review, and pain/progress assessment

Followup Office Visit (complex) -- $125
Complex followups include new problems, pre surgical education, detailed imaging review, and complicated bandage changes.

Established Footcare visit -- $55
Professional care to trim and debulk toenails and shave calluses. Also will evaluate foot type, shoes, and risk of infection

Custom Orthotics -- $450

Detailed and individualized customization of orthotic inserts to improve foot and ankle alignment, improve performance, and reduce fatigue and pain. Dr Patel will go through every component of the orthotic and tailor the orthotic inserts to your needs and your desired shoetype

XRAYS - Foot and Ankle -- $40 one foot ; $60 both feet
Performed in office with mobile-type Digital Xray Unit. To evaluate bones, joints, spurs, foreign bodies, and biomechanics of your feet

MSK Ultrasound -- $50
Handheld and portable ultrasound to obtain live imaging of soft tissue + joint to diagnose pathology and guide injections safely
