:: United Healthcare (Out of Network) - we do not bill United as a primary insurance, but we can give you a superbill for you to contact your plan for self-reimbursement

NOTICE: EFFECTIVE Jan 1, 2026, a $75 no-show fee may be assessed for missed appointments for same day cancellations

***$75 After Hours Fee will be added to appointments made during non-regular hours.
